After you have completed the form and provided an electronic signature, please make sure to press the Submit button at the bottom to automatically send us your information. The security and privacy of your personal data is one of our primary concerns and we have taken every precaution to protect it.
How Does Online Registration Work?
Once you have completed and submitted your form online through PBHS Truforms, our administrative team will automatically be notified and will be able to access and view your records via our secure practice management system so that we will already have your records when you arrive at our office. Registering online reduces paper waste, saves you time at our practice, and allows us to see patients in a time-efficient manner.
