What to Bring

 

REFERRAL SLIP:

Your dentist or doctor will complete a referral slip and either mail, email or give it to you to bring to your appointment. This provides us with the correct information about the treatment you need.IMG_0750

X-RAYS:

If your dentist or physician has taken x-rays, you may request that they forward them to our office by mail or electronically. If there is not enough time, please pick them up and bring them to our office. Sometimes the films are inadequate for the type of procedure necessary. If additional films are necessary, they can be taken at our facility.

MEDICAL HISTORY AND MEDICATIONS:

Please bring a list of medications, vitamins and herbal medicines that you are presently taking. Please alert the office if you have a medical condition that may be of concern prior to surgery (i.e. diabetes, organ transplants, artificial heart valves and joints, rheumatic fever, etc.) or if you are currently taking any medication (i.e. anticoagulant therapy, blood thinners or immunosuppressants, antirejection medication, bisphosphonates, etc.)

INSURANCE

Please bring your insurance information with you so that we can expedite reimbursement. The following information is essential:

  1. Name of insurance company
  2. Address where claims are processed
  3. The policy holder’s name
  4. The policy holder’s place of employment
  5. The policy holder’s date of birth
  6. The policy holder’s social security or policy ID
  7. Policy group number

Although we work with all insurance companies to maximize your reimbursement, we may be considered ‘out-of-network’ for your insurance company. We recommend that you call your insurance company and ask what your plan covers for your oral surgery treatment prior to your visit.

Medicare Patients:

Please be advised that our practice has “opted out” of participation in the Medicare Program. Patients with Medicare coverage are fully responsible for payment of charges incurred at Reynolds Oral & Facial Surgery and claims may not be submitted to Medicare by the patient or our team. Medicare limits do not apply to what Reynolds Oral & Facial Surgery may charge for items or services furnished. 


 

Initial Consultation

 

Consultation

Your initial appointment will consist of a consultation explaining your diagnosis and treatment options. At your consultation visit, you will first view a short video and then our doctors will review your medical history with you and give you a problem directed examination. He will then discuss your personal surgical needs and make the best surgical recommendation for you. A pre-operative consultation and physical examination is mandatory for patients undergoing Intravenous (IV) anesthesia for surgery. Occasionally, surgery can be performed the same day as the consultation. However, a complex medical history or treatment plan will require an evaluation and a second appointment to provide treatment on another day. If we have made your appointment for both a consultation and surgery on the same day, do not eat or drink anything, even water, 6 hours prior to your surgery. Also see our preoperative instructions for the specific procedure you are scheduled for and the intravenous preoperative instructions. IMPORTANT: All patients under the age of 18 years of age must be accompanied by a parent or guardian at the consultation visit.
