Full Bony Impaction

Full bony impaction; prophylactically preferably at an early age to prevent possible pathology.

A. These are typical wisdom teeth that are ready to be extracted. Note that the teeth are still completely beneath the bone and the roots are about 1/3rd to 2/3rds formed. There is not enough room in this jaw to fit the wisdom teeth in a healthy way. Note how, on the lower jaw, the bone swings up in the back leaving no room for the wisdom teeth.

B. 14 year old wisdom teeth with 50% root formation, perfect time to extract.

C. 20 year old full bony impacted wisdom teeth with 1/3 root formation with inferior alveolar nerve proximity. Mesioangular impaction #17 and 32. The position of these teeth will not improve with root formation, it will worsen. Don’t wait for the teeth to erupt. Teeth #1 and 16 have enlarged follicles which would be considered a cyst, which is probably pushing the tooth higher into the maxillary sinus.
