Consultation
Your initial appointment will consist of a consultation explaining your diagnosis and treatment options. Your consultation visit is approximately 20 minutes. You will first view a short video and then Dr Reynolds will review your medical history with you and give you a problem directed examination. He will then discuss your personal surgical needs and make the best surgical recommendation for you.
A pre-operative consultation and physical examination is mandatory for patients undergoing IV anesthesia for surgery. Occasionally, surgery can be performed the same day as the consultation. However, a complex medical history or treatment plan will require an evaluation and a second appointment to provide treatment on another day. If we have made your appointment for both a consultation and surgery on the same day, do not eat 6 hours prior to your surgery. Also see our preoperative instructions for the specific procedure you are scheduled for and the IV preoperative instructions. IMPORTANT: All patients under the age of 18 years of age must be accompanied by a parent or guardian at the consultation visit.

It is not necessary to fill out a health history form for every visit, but please inform us of any changes since your last visit.

It is important that you have at least version 5 of the plugin, in order to successfully use our forms.
