Payment Options:
Payment in full is expected at time of service. To help you meet this expectation we offer 5 different payment options. Please review the following options for a plan that will suit your surgical needs.
Discounts
- 5% Discount for full payment in cash at time of service. (Cash only, checks are given a 3% discount)
- 3% Discount for full payment with Visa, MasterCard, Discover or with a check at time of service.
(Note; we process checks through the TeleCheck service which treats your check like a debit card)
Credit/Loan
50% payment at time of service with Visa, MasterCard, Discover or TeleCheck.
50% payment at 30 days from date of service with 1.5% interest. This second payment is guaranteed with a postdated check or withdrawn automatically on your Visa, MasterCard, Discover.
1/3rd payment at the time of service with Visa, MasterCard, Discover or TeleCheck.
1/3rd payment at 30 days from date of service with 1.5% interest. This second payment is guaranteed with a postdated check or withdrawn automatically on your Visa, MasterCard, Discover.
1/3rd payment at 60 days from date of service with 1.5% interest. This third payment is guaranteed with a postdated check or withdrawn automatically on your Visa, MasterCard, Discover.

Insurance
Dr Reynolds is affiliated with a limited number of insurance plans. However, we will file insurance for you no matter who your insurance is with. We work with all insurances and try our best to recover benefits for you. We have the insurance reimburse you directly. If they mistakenly send the payment to our office, we refund the amount to you. Please bring your insurance information with you so that we can expedite reimbursement. The following information is essential:
- Name of insurance company
- Address where claims are processed
- The policy holder’s name
- The policy holder’s place of employment
- The policy holder’s date of birth
- The policy holder’s social security or policy ID
- Policy group number
